How to fill it correctly
- Choose the consulate for your state (use the jurisdiction finder).
- Fill the form online via e-application, or download your consulate's PDF.
- Complete it clearly — all details must match your passport.
- Print a high-resolution copy and sign it in front of the consular officer (never sign in advance).
Important: you must sign the form in front of the consular officer. Signing in advance can invalidate your application.
